ALL STUDENTS REQUESTING A WORK PERMIT

Follow these simple steps:

v  GET A JOB OFFER FIRST, then

v  Obtain a B1-1 Form, “Statement of Intent to Employ Minor and Request for Work Permit” from the Work Permit office in room 74.

v  Fill out the form completely and properly (PRINT); obtain signatures from your employer and parents.

v  Return the application to the Work Permit office in room 74.

v  If the Work Permit office is closed, ask an AP secretary to place the form in Mr. Reedy’s mail box. Return the next morning to the Work Permit office to obtain your work permit.

v During the summer or any other time you need a work permit you will need to make an appointment with Rene Copenhaver (909) 580-5000 ext. 6537 at the District office.   You then go to the District office which is located at 1212 Valencia , in Colton .   Take 10 East to Rancho off ramp, turn left (north) to Olive, turn right on Olive then   left on Valencia .  The District office will be on the right two blocks north.
