Complete the form.  Obtain the proper signatures.

 

Take the form to one of the Work Permit offices below on the day they are open or call the person listed below at the District office to obtain an appointment.

 

The work permit office at Bloomington High School will be open on Tuesdays June 17, July 1, July 8 and Thursdays June 19, July 3 and July 10 form 8 am to 10 am.

OR

 

The work permit office at Colton High School will be open on Wednesdays June 18 through July 23 between the hours of 8 am and 10 am.

OR

You can go to the District office at 1212 Valencia Avenue in Colton between 7:30 am and 11:30 am Monday - Friday if you make an appointment with Ms. Copenhaver.  Her number is (909) 580-5000 extension 6537.
